Instrumented Soft Tissue Manipulation

Instrumented Soft Tissue Manipulation In osteopathy the concept of soft tissue dysfunction, or in English the muscles, fasia, skin and ligaments can produce pain and disability when there wouldn’t appear to be anything wrong on xray or scan. Problems that occur can be due to scarring, infection, prolonged immobility or injury that involves swelling and inflammation. Inflammatory fluid is like a weak glue that perculates through the tissues and sticks them together forming adhesions. From there on muscles may not slide across one another, nerves get stuck down, joints become stiff and pain results. Historically massage or rubbing of painful parts with bone / or stone has been used to provide relief. Osteopaths have predominantly used their hands but based on an ancient Chinese called Gua Sha, rubbing or scraping of the skin with blunt edged bone has progressed to a modern “high tech” form of soft tissue instrument. The most commonly known is the Graston Technique with its patented tools though others such as Myiodac produce similar ones. These are specially shaped high quality stainless steel instruments which can be used to firstly diagnose scar tissue or little knots in muscle and secondly to treat by breaking down of scars and ironing out muscular knots. The instruments act as a “soft tissue stethoscope” whereby any problem is picked up during a diagnostic sweep of the instrument over the skin. The resonance is then relayed and amplified through the steel to the practitioner who can then concentrate treatment to the problem. Treatment would involve cross fibre strokes, stretches and to produce a local histamine response which in turn creates a localised hyeraemia of inflammatory fluid to provoke a healing response. The tools are so shaped to go around most body parts. The technique can be uncomfortable but in context momentary discomfort compared to pain and disability of the part is well worth the outcome. Certainly the results would appear to be superior to conventional massage or manipulation.
